The most common failure in a second round is that the real objection quietly disappears because the draft changed enough to hide it. Carried pins are the mechanical fix for that.
Every decision is recorded
Approvals and change requests attach to a specific version, with the notes that produced them. When someone asks what was signed off and when, it is a link rather than an email search.
Clients get a lane, not an account
The client role sees the work sent for review, their own notes and the sign-off history. Not your chats, not your other clients, not the machinery behind it.

Producing work bills as normal; reading it, pinning it, arguing with it and approving it never does, and a client can never be blocked from signing something off because a balance ran down.
